Hello!

macOS High Sierra 10.13.1;
SoftRAID Lite 5.6.3;
Four SAMSUNG MZHPV512HDGL through PCI Express Gen 2 Carrier Board for M.2 SSD modules http://amfeltec.com/products/pci-express-carrier-board-for-m-2-ssd-modules/

Softraid unable to create raid-0. Why?

You apparently have a reliability problem. that disk is not staying connected, or is failing to "report in".

Try fixing all the connections solidly. its possible this board is not reliable in OS X, or is a faulty card. Disks (SSD's) should not do this.
