I have a two-disk mirror, and one of the disks died. I have put in a new disk, initialized with SoftRAID Lite, but the Volume, Add Disk... menu item is greyed out.

What do I need to do to be able to add a disk to a RAID volume?

Are you selecting the Mirror volume tile?

Yes, I'm selecting the Mirror volume tile. Ctrl-Click presents me with the same as the Volume menu.

Everything in the menu is greyed out except "Unmount" and "Disable Journaling". The "New..." menu item is also available in the Volume menu (but not, obviously, in the mirror tile's popup).

This must be something very simple. Send us a tech support file at support@softraid.com and we will post the answer here.

Is it the case that SoftRAID can't operate on Apple RAID-create arrays? I ended up having to just create a new mirror using SoftRAID, and now all the options are available.

For SoftRAID to manage disks/volumes, the disk needs to be in SoftRAID format, so the driver can control the disk.
